I have seen this type of holder used as promos, but never could find the directions. I did finally see a diagram so thought I could make an svg for cutting it. I've been working on this off and on over the last few weeks. From some of my test runs, I find that it works better with stiffer cardstock. I'm not real happy with the closure; it will need to be reinforced to keep it from tearing. But, all in all, it does work.
